Do you need a blood glucose monitor and need to learn how to use it to test your sugar levels?

  • Would your health benefit from losing weight?

  • Do you want to avoid nasty complications from diabetes?

  • Are you looking for extra support, or has there been a change in your medications?

  • Do you want to feel empowered by understanding your diagnosis and what you can do?

    You can visit a nurse practitioner as many times as you require, and you will always receive a Medicare rebate. Nurse practitioner visits do not use any of the Chronic Disease Management Plan visits, they are separate.
